There are several clinics specially for looking after patients with ongoing conditions such as heart disease, diabetes, high blood pressure, asthma and other breathing conditions. You will be sent an appointment to attend if it’s appropriate, but if you feel you need to be seen beforehand, this can usually be arranged. Specially trained practice nurses mainly run the clinics. If you are usually seen for more than one chronic condition it is possible for you to be reviewed in just one appointment, please ask reception when making your appointment.

Should transgender men have cervical screening tests?
Cervical screening checks the health of cells in the cervix. Detecting and removing abnormal cervical cells can prevent cervical cancer.
Cervical screening is available every three years at ages 25 to 49, and every five years at ages 50 to 64.
Trans men who still have a cervix are entitled to have cervical screening. If you are:
Trans men (individuals who have changed gender from female to male) who have had a total hysterectomy do not need to have cervical screening tests.

If you are asked to provide a specimen by your GP or nurse, it is vital that the pot is clearly marked with your name and date of birth. If we are not able to identify you, the specimen will be disposed of. If your specimen needs to be sent to the hospital for testing, please bring it to the surgery by 11:30am. If you have been referred for ear syringing it is mandatory for you to complete a consent form before the nurse will carry out the procedure. This is to ensure that you are aware of all the risks associated with ear syringing and to inform the nurse if you have had any previous problems. Please complete the form and bring it to reception to obtain an appointment with the right clinician. An appointment will not be given without a completed form. Please note if you tick yes to any of the second list you will not be able to have your ears syringed at the surgery without having a prior check with the nurse. A referral to the Ear Cleaning Clinic may be necessary.
Most of the doctors are trained and experienced in performing minor surgery including the removal of skin lesions, joint injections and cryotherapy (treating skin lesions with liquid nitrogen). These appointments are limited so there is usually a waiting list.

The District Nursing Team plays a vital role in patient care in the community, adapting to meet new challenges and taking on new responsibilities. It is our aim to provide you, the patient, with the best possible care.
The District Nursing Team in this area are employed by, and are responsible to, Dorset Healthcare University Foundation Trust. You or your family/carer can ask to see a member of the District Nursing Team direct or through your GP, Health Visitors or Social Services.
The District Nursing Team will accept referrals when the patient has a clear need for a complex assessment of health needs and/or nursing intervention and:

Lucy Lillywhite is the midwife based at Wareham Surgery, and is part of the Purbeck Team of midwives. The Purbeck Team offers a range of services throughout your pregnancy and postnatally, including:
Midwifery clinics at Wareham Surgery are on Tuesday afternoons 2pm – 5pm (telephone 01929 553444) and at the Children’s Centre (telephone 01929 552864) on Thursdays at 2pm-4pm or 5pm-8pm (appointments necessary for both clinics).
Community Matron: Jodi Nicol
The community matron is a senior nurse supporting people with long term conditions, mainly aged 65 and over. She/he is able to work closely with the patient and their doctor to plan and organise care. She/he is also able to co-ordinate support from other agencies, such as voluntary services, etc, thereby reducing visits to the doctor’s surgery or hospital. She/he can make sure you understand how your medicines help you.
People with long term conditions find themselves admitted to hospital because their symptoms have deteriorated. The Community Matron gets to know the patient and spots difficulties as they arise and makes sure the appropriate care and medication are provided.
Community Matrons have small caseloads to enable them to provide a comprehensive service. The caseload is determined in discussion with GPs and other health professionals.
The surgery no longer has a dedicated counselling service. Psychological therapies are provided by Dorset Healthcare NHS Trust via referral from the GP or self-referral. Self-referral forms are available in the waiting room, or click here to download.

This is available through referral by your doctor. There has to be a medical need e.g. diabetes, the service is not available solely for nail cutting. Once you have been referred to the podiatrist you will receive notification of your appointment by post. Podiatry Clinics are held at Wareham Hospital. The surgery offers appointments with the Practice Nurse for travel immunisation advice. A questionnaire must be completed prior to an appointment being given. Please collect the questionnaire from the main reception or click here to download the form. Please note, you must be a patient registered with the practice.
An appointment will be made after completion and return of the pre-assessment form. Due to high demand and staff shortage there is a significant delay in the service, so please allow plenty of time for foreign travel advice. You may require a course of vaccinations which have to be ordered in. We are currently unable to accommodate patients travelling in the next six weeks. Please note forms are processed in order of departure date. If you require urgent travel vaccinations or do not wish to wait you are advised to attend a private travel clinic.

Private fees will be charged according to the British Medical Association recommendations that apply at the time for services such as the following: Private sickness certificates, private health insurance claim forms (e.g. BUPA,PPP), driving medicals, school fees insurance, fitness for occupation, travel or sporting activities, solicitors reports, travel medical forms, etc. An application form is required for all requests. Please enquire at the reception desk regarding the appropriate fees, or click here and please provide a stamped addressed envelope if you wish the forms to be posted to you on completion. We require payment before you receive your form. Please note we are able to take payment by credit card but we do not accept American Express.
Please give as much notice as possible if you wish to book an appointment for a private medical examination, e.g. driving medical. We will require the forms prior to arranging an appointment.
