Anyone living in the area covered by the surgery can be registered. To register, simply ask at the reception desk. If you have a medical card this will help, but is not essential. You will need to complete three forms; one provides the information to register you with the surgery, the others give us useful background information concerning your health, and who we may contact as your next of kin. Once registered with the surgery you can arrange to see any doctor, even if you usually see a different one.
Every registered patient is registered with a named accountable GP. It is important that you stick to seeing the same doctor as far as possible, particularly if you have an ongoing problem with an aspect of your health, so we will try to organise appointments to allow this. To do this we may need to ask you whether your condition is new or ongoing. It may be necessary for you to wait several days and sometimes longer to see the same doctor, but we believe this is in your best interest. If you see a different doctor just because there is an available appointment it can sometimes lead to a different approach to your treatment, which could lead to subsequent misunderstandings. Sometimes you may get a telephone call from your doctor when there are no immediate appointments to plan your treatment.
When your doctor asks you to make a follow up appointment, please do so before you leave the building, or as soon as you can.
If you see a different doctor, please be assured that he or she will have access to your medical records and will record any treatment and advice given to you.

Practice area: the wards of Wareham Town, Wareham St Martin and Creech Barrow (Arne, Church Knowle, Kimmeridge, Steeple, Tyneham). These are for all new patients who have recently joined the practice. We’d encourage you to take advantage of this opportunity for screening and a general discussion about your health. It is also an opportunity to ensure that we have details of any medication you might be taking regularly, and these are put on your records before you start to run out! Please make an appointment with the Practice nurse.
New arrangements introduced from January 2015 give people greater choice when choosing a GP practice. Patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.
GP practices have always had the ability to accept patients who live outside their practice area. Regardless of distance from the practice, the practice would still provide a home visit if clinically necessary.
The new arrangements mean GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.
Out of area registration (with or without home visits) is voluntary for GP practices meaning patients may be refused because they live out of area. With effect from October 2016 no new registrations from outside the practice area will be accepted. The practice will consider applications from patients currently registered at the practice who are moving out of the area but if accepted the patient will have to re-register as an Out of Area patient by completing a new set of registration forms.
If your application is considered the GP practice will only register you without home visits if it is clinically appropriate and practical in your individual case. To do this we may:

If accepted, you will attend the practice and receive the full range of services provided as normal at the surgery. If you have an urgent care need and the surgery cannot help you at home we may ask you to call NHS 111 and they will put you in touch with a local service (this may be a face to face appointment with a local healthcare professional or a home visit where necessary).
We may decide that it is not in your best interests or practical for you to be registered in this way. In these circumstances we may offer you registration with home visits, for example, if you live just outside the practice area or we may not register you and advise you should seek to register (or remain registered) with a more local practice.
If accepted but your health needs change we may review your registration to see if it would be more appropriate for you to be registered with a GP practice closer to your home.
